Application/Order Form

To apply for classification of a TVC or infomercial, you will need to complete the relevant Application/Order form.

CAD has recently combined the Application form with the Order form.

There are two tabs contained within the document; the first is the ‘Application’ form; the second is the ‘Order’ form. Both forms need to be completed.

1.	Simply complete all fields on both the Application and Order form and print.
2.	The Application form needs to be signed and the Order form needs to be signed if you are paying by credit card.
3.	Forward your Application and Order form along with a detailed copy of the script/storyboard and any relevant documentation to Commercials Advice.

Classification cannot commence until CAD has received all your materials including a copy of the commercial.
